It is a printable PDF. It has to be signed and four of the store policies initialed, so it cannot be filled out as a web form.
Missing documents are the most common reason an account sits unapproved.
- W-9 or other proof of business / Federal Tax ID
- Signed Texas exemption or resale certificate — only if claiming tax exempt
- Texas driver’s license and EPA 608 card for each person who will buy refrigerant
- Sample purchase order, if your company requires one
- Property list, if the account covers more than one location
Email the signed application and documents to parts@otp-hvac.com, or bring everything to the counter at 10226 Plano Rd, Suite 104. Applications are reviewed and approved or declined, typically within one week.
